Experiences with a Life-Sustaining Treatment Decision System: Focus group interview

Abstract
With the aim of establishing a life-sustaining treatment decision system over the past five years, various studies have been conducted to identify the possible outcome(s) of the system. An appropriate strategy for the future is required as the current system has been widely implemented and changes in actual care need to be considered. To this end, the experiences of medical providers and medical users who have been at the center for using the system are valuable data sources. The purpose of this study is to suggest strategies to prepare for the future of the hereby discussed life-sustaining treatment decision system, which characterizes the current situation based on experiences related to end-of-life care through a focus group interview and draws conclusions from it for the future. The interviews were conducted for three respondent groups (institutional workers, medical providers, and medical consumers), focusing on changes in the overall social environment including death and dying culture, predictable system changes, and coping strategies. Results of the interviews were analyzed using the thematic analysis method (Braun & Clarke, 2006). Study respondents consisted of six institutional workers, four medical providers, and five medical consumers. Nine sub-categories and three main categories were identified based on their interviews. Main categories were ¡°Lack of providing accurate information on the life-sustaining treatment decision system,¡± ¡°A clinical environment that is passive in using the system,¡± and ¡°Gaps caused by system deficiencies.¡± This study has important practical implications for suggesting the direction to develop a life-sustaining treatment decision system through the voice of the field. To improve the system, it is necessary to establish specific goals and plans in terms of systems, services, and socio-cultural aspects.
